Take the baby’s pictures (eye open), filled out DS-11 passport application, a birth certificate, the baby itself + its two parents and the applicable fees to one of the US post offices that issue passports (or a US embassy if you’re overseas). Yes. One minute on google tells me that the cut-off seems to be 15 years. So you’ll need to apply in person, using the same form as a new applicant: Renew by Mail

Can a family member or friend submit a passport application on a person's behalf?
No one can submit on any body else’s behalf. It means an applicant acting as X cannot apply on behalf of Y. (except as parent of a minor). One can though help in filling the application with all the details of the applicant.Specifically one cannot specify that s/he is applying on behalf of her/his mother/father/brother etc. The person in whose name the application is made shall have to appear in PSK himself, give own biometerics/picture etc.
