How can I request an U.S. passport for a baby?
Take the baby’s pictures (eye open), filled out DS-11 passport application, a birth certificate, the baby itself + its two parents and the applicable fees to one of the US post offices that issue passports (or a US embassy if you’re overseas). If my US passport is over 15 years expired, will I be considered a new applicant?
Yes. One minute on google tells me that the cut-off seems to be 15 years. So you’ll need to apply in person, using the same form as a new applicant: Renew by Mail
